Hans Faber starts the first day of a year-long shadowing at the Van der Hoeven Kliniek, a TBS clinic. TBS patient Samira has just come out of the recovery room where she was because she had used drugs. Patient Suzan shows Hans the dents in her head that occurred after her brain surgery. Since that procedure, she has less compulsion to attack people. TBS patient Kees works in the administration and prepares the morning bulletin. He takes notes of what happens in the clinic and he is not always appreciated for this.

Hans Faber is surprised that patients in the TBS clinic are allowed to borrow porn DVDs and also wants to know why drugs enter the clinic despite checks. TBS patient Kees goes to the prison in Vught with an escort to welcome a new patient. TBS patient Milly tells Hans about her budding relationship with a fellow TBS patient. The app traffic between TBS patient Ben and the women he meets via a dating app is monitored.

TBS patients are held partly responsible for safety in the clinic. When an aggressive TBS patient knocks a work supervisor unconscious, it has an impact on both TBS patient Renesto and the employees. The relationship between TBS patient Milly and a fellow patient is broken by the clinic because it stands in the way of their treatment. Hans hears that the male TBS patient is therefore being transferred to another TBS clinic.

Hans accompanies six employees who bring food to a TBS patient in the segregated room. Workmaster Fred goes on a sailing holiday in Sneek with a few TBS patients and colleagues. Hans looks back with him and discusses what thoughts he has about this holiday. Patient Samira is allowed to go outside for the first time after five years and would like to go to the supermarket during her supervised leave. TBS patient Kees continues to refuse hormonal libido inhibition and knows that he cannot therefore go on unescorted leave.

TBS patient Samira went crazy and was transferred to a higher security clinic in the Netherlands. Supervisors tell Hans about the fear they felt; afraid that Samira would do something to them or herself. TBS patient Ben, who had moved to live in an apartment outside the clinic after years of TBS, was brought back within 24 hours because he had consumed alcohol. Was he ready for a return to society?
